Has the Halifax lost the plot

I would welcome any advice/comments please.
My sister has had a current and savings account with the Halifax Bank for years. she also has a Halifax Credit Card. Yesterday her husband received a letter from the Halifax Bank with a new pin enclosed (he does not bank with the Halifax and has no connection with them)
My sister phoned the Halifax to be told that a new credit card was being sent to her husband as an additional card holder on her account, she has never requested this or has her husband, the Halifax even had all his details as well.
The Halifax are adamant that she requested the card she is equally adamant that she did not request a card and has not even spoken to the Halifax in the last year or so.
To add insult to injury the C/S at Halifax were very rude to her and she is livid to say the least.
Any advice please, has anyone had a similar experience to this, I certainly have never heard of a card having been sent without a request for one.
Aiming to be debt free....but still off target
