Help writing a letter of complaint

Unfortunately, I had to stop at some Motorway Services the other day.
I encountered a couple of problems:
1 - the automatic door wasn't working, the other doors were all shut. They were heavy doors and there were no staff around.
2 - The disabled toilets required a radar key, but I couldn't find any staff to allow me access
3 - Both lifts were out of order
4 - The toilets were on the first floor, so to use a bathroom, I had to climb stairs.
Now, I managed it (with a great deal of pain) because I only have temporarily restricted mobility. Because it is temporary, I don't have a radar key.
I don't think this is acceptable. The Out-Of-Order signs on the lifts were very old and dirty, and I wonder if they are ever going to bother to fix them.
I'm not sure how to word a letter to the Services manager, but I feel I really should complain. Luckily I managed, but only with pain and difficulty. Other people might not find it so easy.
Any advice?
