MBNA Issues - please advise

Hi Guys,
I have a couple of problems which I hope you will be able to help me with.
For the past year I have had some debt on 2 credit cards (under £3k in total) which I have now almost paid off in full.
Last month, after receiving my monthly wage, I attempted to pay off the remaining balance on my MBNA card (£720). On trying to pay this amount, the MBNA website gave me an error saying that my debit card had not authorised the payment. I tried again - it did the same.
A few days past and on checking my Lloyds current account I realised that MBNA had in fact taken the money from my account - twice. So I was now down £1440 and after paying other bills for that month I had a bank balance of.... Nil.
Long story short - managed to get the money back off MBNA (but not without alot of shouting and some inept call centre employees) but by this time it was too late and other arranged payments had been taken from my current account.
Today Lloyds TSB have charged me £105 for going over my overdraft limit last month courtesy of MBNA. What I want to know is, how do I go about claiming this money back off of MBNA? Obviously writing a letter of complaint would be the first thing to do, but what are the chances they will pay up without a fight? What would you advise?
Also in addition to this, around a year ago I requested my credit card account history from MBNA with a view to reclaim my charges. I sent this off and they denied it (I think it was about £500) but for several reasons I won't go into I did not follow this up. After all this time, can I now re-request my account history and try to reclaim my charges - which will of obviously gone up a bit by now?
Apologies for the long mail - but I am really livid with MBNA and I want to explain in full what has happened so that hopefully I can go down the proper channels to get these issues sorted out.....!
