Free Prescriptions For Cancer Patients (England) Act Now!

I just got the latest email from Martin and it had this useful bit of information in it:
"From 1 April 2009 cancer patients can also get free prescriptions. Apply before 24 March and you should get your exemption certificate in time to use it from that date. Apply after then and you'll need to claim back any prescriptions you've paid for since 1 April.
The certificate is renewable every five years and won't be claimed back if the condition changes. If you're a cancer patient currently getting free prescriptions based on low income then it's worth applying for this instead. Application forms can be picked up at your surgery or at the oncology department."

Fantastic! but when I asked for the form at my GP surgery they hadn't received them even though the forms were supposed to be sent out by the 9th February. They even suggested I try another surgery in case they had received some!!

Tried the number, they passed me onto local prescription service who gave me a different Newcastle exemption team number who said I should get the form from my GP (duh) so I rang them again. The lady who deals with forms was off!! A message was left to ring me the following day (yesterday). Still waiting!!!!
Wot on earth do I have to do just to get something I am entitled to?
