Hardship Reclaim !!! Help Plz

I recently sent off my bank statements to halifax claiming bank charges of £6,000 however i got the usual letter back saying these charges were lawful etc. I would now like to go down the road of claiming through hardship.
Is there a template letter for this? I`m sure I seen it and now can`t find it ( typical huh ? ) if not does anyone have any tips what to put in the letter and do I have to send off anything else?
