lost item form

someone recently posted a link to a form that you get a buyer to fill out when they are claiming an item didn't arrive, it think it was stevew8975
does anyone have the link please? I have searched but can't find it.
i have currently got a buyer claiming non delivery, their first message asked for a tracking number when they chose to pay for standard 1st class, 2nd message asked if i had proof of postage and "i dont know what the next step is i suppose i get refunded by paypal" i said i would refund if it didn't arrive and asked them to wait until friday 6th feb, but today i have got a message asking for a refund asap. this is also not the first time that an item they purchased has not arrived. so i am a bit suspicious as they don't have much feedback.

Generally I'd agree, but there are a growing number of chancers out there that should be made to jump through an extra hoop to receive their ill-gotten gains.
It's well documented about the number of missing items that miraculously appear when any mention of a fraud investigation is raised.
A Lost In the Post declaration will not take precedence over any Paypal claim, and chancers often think twice about putting their name to a false declaration that could well be used by the RM in any investigation.
In my experience, genuine customers who have not received their item have no hesitation about filling one of these in.<--- Nothing to see here - move along --->0 -

i will be refunding the buyer if they are adamant that they have still not received the item. paypal don't need to get involved.
if i get a negative after refunding the buyer for an item lost in post then yes i would be annoyed.
i have been using ebay for 7 years and have refunded buyers on numerous occasions for items lost in post. i just think that this person might be trying it on so would like to give them the opportunity for the item to 'arrive' before i refund.
note to moderator/board guide - this thread can be closed now as i was just asking for a link to the lost item form and that request was fulfilled in the 2nd post.
