Questions on PPI for credit cards

Despite the fact that a number of years ago I had declined PPI somehow it managed to creep back into my cards as a charge. At the time I had been extremely busy with Uni etc and was hard pressed for cash but somehow managed to not see the charge! Naturally paying off any debts on the cards were a struggle.
One of the cards was closed a number of years ago but I still have some debt remaining on another card.
My questions are these.
How long back can a claim for PPI be made to?
Can a claim be made on a card that no longer exists?
If it can and I only have statements from 2001 is there any way of claiming back PPI payments made before this time?
What risk is there in the banks closing the credit cards and/ or bank accounts if a claim is made?
Things have been a struggle the last 7 years or so as I did not immediately get a job again following Uni.
Thankfully I have managed to get many of my interest rates lowered and opened a new account with 6.5% for the life of the balance. So far so good.
Any advice would be appreciated.
