    Relax! There is nothing to worry about, when you go in tell them that they have made a mistake on the form and that you left on whichever date you actually did, they will make the amendment and print the forms off again, that'll be the end of it, you need to do this as they will write to the employer to ask when you finished and why.

    They will take your claim from the day you rang up anyway so there is no fraud involved, its just a simple case of amending the end date.

    What have you been living on since November?

    You should get a doctor's note to say you were not well during that period and tell the truth. You could could have put in a claim for Incapacity Benefit/Income Support if your doctor agreed you were unfit for work at that time. If you lie you are putting yourself in a worse position than if you tell the truth as you might get sanctioned for leaving the job.

    Good luck.

    You gave into the temptation to lie about leaving but are doing the right thing now. I dont agree with the first reply that states you have nothing to worry about but I hope the decision maker understands your situation.

    Even if your doctors change regularly they should still have your history and be able to give you some sort of medical report or medical certificate to cover you. I know some might charge for those reports but in the long term, if it keeps your JSA right, then it would be worth it.
