probably been asked before

My folks have started claiming back their charges and have got to stage 3 where they got the standard"charges are fair, test case, blah, blah, blah"letter but they are unsure as to the next step. They are claiming under financial hardship(mum got made redundant a few months ago and dad is claiming DLA) and got the form from the bank included in the standard response letter so the question is this,

do they fill out the form and send that back solely or is there another template letter that should go with it or anything else that they should send?

Im not sure myself and dont want to give them duff info.

Thanks in advance.
