I have just received my AMEX card. My question is that does my 5% cashback for 3 months start when AMEX sent me the card or when I activate it? Or some other time?

All the deals/promotion starts from the date you have been accepted (i.e. signed your agreement online or on paper).0

It's when your account is opened which is usually when you have been accepted (as mentioned), not when you activate the card so don't delay!0
