I need a bit of help

A while back I was doing supported permitted work (work which met required hours/pay conditions and I received a lot of supervision from a supported employment worker). My benefits weren't affected except IS which was reduced to £20 per week.
I have now left said work and I am unsure how to get my IS raised to its original amount (I also receive DLA so my IS has a "bonus" added). I no longer receive wages and I still get £40 per fortnight IS.
One person told me I need to fill in an A15 form whatever that is someone else said I just need my final pay slip. And who do I send this to the job centre or the DWP?

its jobcentre plus nowardays,
just write a quick note to the address on your benefit letter and inform them of your change of circumstances (which you are obliged to do). Benefit letters often come with a change of circumstances form which you could use instaed. They will write with copies of the correct forms if they want them and request any evidence,0 -
