Bus pass and "DBD 384" form

Which one is the DBD384 form? Can't I just send any letter that proves I am in receipt of x component and rate of DLA like the entitlement summary?

i just sent them the first page of my letter from dla confirming which rates i was on. i went in person though so don't know about the photocopy.0

if its for the council most don'e accept photocopies, normally you can send your original (I stic on a post-it note asking for it to be returned to my address). or you can take it in an they will photocopy it and give you a recipt.

dbd348 is your dla letter of entitlement (just the front page should do it).
If your worried about loosing it photocopy or scan it in before you send it, but it is easy to get a spare copy if you ring the dwp they'll stick one in the post for you (I found this out as they spelt my name wrong on the last one!)0 -
