Does anyone know for certain if it is cheaper to turn on the immersion for an hour before you need it, eg for a bath, or to keep it on all the time?

Only when you need it especially if its not thermostatically controlled.
If you left it on all the time even with the best insulation available today you will still experience heat loss from the cylinder through conduction meaning you are throwing money away.0
