    Thanks - as it isn't particularly clear what to do to download it:

    1) Click on the "Download with filefactory basic" link (about 2/3 down the page)
    2) enter the 4 digit code, click 'continue' button
    3) click on the green 'begin download' button
  • Thanks for the link. I can't seem to get hold of this file - the site keeps on telling me it is busy. Any ideas?

    Got the files now,thank you. Was there anything specific to your accounts with these forms?
    I need to claim for POA accounts and the sooner the better so if nothing else included in the pack you got, I don't see why I can't start the process now.
  • Finally managed to get hold of the files after trying half an hour later!

    I was hoping we could print fill these in and send them off now to save time but it looks as though there is a unique barcode on page 1?

    Gerontius mentioned in another thread that there was a pre-addressed envelope in there
    Incidentally, the Post Office has no record of the FSCS being at the prepaid delivery envelope address at Warehouse 3, Faverdale Industrial Estate, Darlington, DL3 0QN for recorded delivery. I've checked with customer help-line who say they've too many addresses to check on the phone
    Guess there's a potential issue if you sent your form to the wrong place??

    I don't know if it is unique. We need someone else to check but I think it is wise to perhaps wait a bit longer. A real nuisance. Will have to make alternative arrangements with the new 4-6 week timeline.

    The implication from the paperwork is that you will receive only one form, even if you have more than one account. I have 2 fixed term accounts that I have opted to go to maturity. It looks like I will have to attach another sheet of paper to the form with account nos. and what I want to happen to each one signed and dated.

    The address is Capita Business Services who specialise in forms handling and data capture, especially for the public sector.

    It is a MicroSoft partner http://www.microsoft.com/uk/experts/solutionfinder/default.mspx?partnerid=4aece42899704f2e8c3b90b99cfc4c62

    Seems quite safe but the bar code could well be an issue.
