Icesave - time to get the money back!

Hi folks
So far i haven't received an email from Icesave regarding getting my funds back.
I would prefer to with draw the funds out via there application form rather than on the web.
Does anyone know how i go about this?

What type of account(s) do you have? (e.g. easy access; ISA; fixed rate)
If you have a fixed rate, do you want to withdraw finds now (and lose interest since 7 Oct) or let it run until the end of its term (and keep all the interest) ?0 -
The FSCS wants to process as many people as possible through the automated online system, so they probably won't be sending out the paper forms until after the 4th December deadline for the online process.0
