My policy wording is slighty different as they have obviously updated but nevertheless still says virtually the same0 - 
            Personally Id be asking for their dismissal procedures etc, surely even within a year of starting they can't simply fire you on a whim claiming incompetence / lack of skills without first notifying you and attempting to resolve the issues they have with your work.

Why not phone your insurers up before filling out form? I would or just fill it all in - you have been paying the money amount to them for long enough! Usually they don;t pay the first 3 months into a new policy, which are fine on.
Do see your concern though - sounds like you was in some kind of sales job or recruitment consultant position - if they put you missed your targets on your offical letter (most emploters wouldn't be that harsh by the way), insurers may say policy is void - read small print or phone them up and probe them verbally on 'theoretica situations on payout' - before filling anything out.0 - 
            definitely ask your manager for a copy of the company's disciplinary policy. i know you have not been there long, but an employer has to try and resolve a problem/give you time to improve etc if indeed there really is a problem.
o/wise anyone could fire anyone for anything or nothing! IYSWIM?

            I would not look at the company policy they should have given it to you when you started (statement of terms and conditions which they have to issue before three months is up) however there is a minium policy that they should of followed. From the sound of it they did not follow, the statutory dismissal/disciplinary and grievances procedures for the uk (i do not know if they have there own procedures over and above that but that is what should be done minium). Were you invitated to a meeting? told you could have a trade union rep or colleague with you? told that you have the right to appeal and who you need to appeal to?
if not pop yourself down to your local job centre and ask for a et1, fill it in and go for a employment tribunal (you will be entitled to claim under one of employment due to them not following procedures).
If you worked for my company (you would have got a probationary review letter (if you were in your probationary period) and i would have collected statements from others regarding your performance, if you were out of your probation period i would have invited you to disaplinary hearing again i would have collected statements etc, under one year you have very little rights unless the correct procedure was not followed when it can become a automatic unfair dismal0 - 
            As far as I know, employers are allowed to unfairly dismiss you so long as you've not been with them for a year (then unfair dismissal legislation kicks in). So going after them for not firing you properly is a non-starter. But if they're a small company, they may well be able to give you a letter that meets your insurer's needs.
