Icesave - Log receipt of forms for Fixed Term Accounts

Could we have a separate thread for people who have opted to retain their Icesave Fixed Term Account to log when they receive compensation application forms from the FSCS?
I've read the e-mail covering this and am still confused as to whether they're going to send out forms straight away for us to complete and return, to be held by them until the accounts mature, or whether they won't be sending the forms out until just before maturity date. Have I missed something?

Is anyone keeping track of progress with the forms? I'm now well into week two with no sign of one.
