DLA Claim Form Example - Rheumatoid Arthritis

i wish i had read this thread before i applied for my dla, i am 26 years old with 3 young children the past year ihave begain to get quite bad to the point now where i find it hard to walk and do everyday things like get out the bed get in and out the bath go to the toilet im in constant pain all the time but i am now worried that i have not put enough in to my claim form loking at your example i havent , i have arthritus in both my knees thumbs jaw both arms one arm also has tennis elbow

I have now used this as a guide to filling in my DLA forms as i wasn't happy with the ones done by my local DIAL. I'm no where near as bad as the OP but they way they have described things is much better than DIAL did.
