Help! Financial Hardship?

Help!
This my sound a little daft BUT i really need help ref this financial hardship thingy. I am about to put in a claim to Lloyds for a substantial amount and as it will be over 3k i have been told i will need to claim financial hardship. How do I go about it? Do i need to include it in the letter or do I fill out a form? I am totally lost!!

    The amount being claimed is irrelevant, financial hardship is based more on whether your outgoings are barely covered or exceed your income.

    And yes you need to stress this in the letter
