Lost DLA form

I have lost my DLA form (I have MS and get the higher component for both) and need it to renew my blue badge. Is there anyway I can get a new one? Thanks.

Do you mean you have lost your award letter? If so just contact the office which deals with DLA - in Blackpool (I think) and ask for a copy of your award letter. Hubby lost his and it was no probs getting a copy.0 -
