It depends on the level of DLA, if you have middle care DLA you can get the severe disability premium if you live alone and no one receives Carers Allowance for you. If you have higher care DLA you can get the enhanced disability premium. If you only get the long term rate (without age addition) of IB and have no other income it is possible to get a small amount of IS too.

yes and no. if your IB is more than you'd get on IS then no. but if your IB is less than you'd get for IS (eg. you have a partner who doesn't work or have any benefits in their own name -- or anything they have added to your IB is less than you'd get on IS) then yes you can get a top-up

And as healy says, dependant on which DLA you get, you may be entitled to extra elements of IS -- which means you have more chance of being eligible for a top up. And once you get even pennies in IS that opens up the door to other things (such as council tax benefit)

We weren't entitled to a top up as such last year, as our joint income was too just above what we'd get for IS. But the combined income wasn't as much as IS plus what we'd have been entitled to for mortage assistance if we were on IS -- so we ended up with 10p/week IS, partial mortgage assistance (also less the 10p/week that came through as IS -- they can't pay MA unless you get something for IS apparently!) and a reduction in council tax.

You only get the severe disability premium if you are on middle or high rate care. If you got high rate care you'd also get the enhanced disability premium. In these circumstances all the premiums are paid

Just high rate mobility then only the dp.
I'll get you, my pretty, and your little dog too!
