Kaupthing Edge/ING Direct Tax Question.

Quite a mundane question for our troubled times but I was wondering if anyone knew where I should send my 'Claiming for Gross Interest on Savings' form now that Kaupthing Edge is ING Direct. I was all ready to send it when Iceland began to melt and I thought they'd have better things to do but as things seems to have settled down again I want to send it off before the next months interest is paid.
(Also, does anyone know if interest will still be paid monthly?)
