Help needed to identify a credit card

I've just applied for a new mobile on orange and been refused which was a big surprise as i've applied for 2 credit cards in the last 6mths without problems.
I applied for my equifax file online and i can't see any probs, the only late payments are 5- 1 mth late payments to empire in 1999. Everything else is up to date but there is a credit card showing on my file from a company called CREATION with a £500 limit taken out in 2000. The account is clear but I have no idea what card this is and need to find out so that i can cancel it. Anyone heard of this company, the only thing i can think of is it must be some sort of store card that I may of taken out to get a discount at some point but don't remember what.

One late payment shouldn't be enough to trigger an Orange refusal.
They may be using a different credit ref. agency though - you have to check both experian and equifax unfortunately.
Have you moved recently? Are you on the electoral roll. Suggest you ask to speak to someone at Orange if there is obviously a mistake.

I have sent off for experian file as well but that one has to come through the post, tried speaking to orange but they just said they don't know the reasons they just get a pass or deny because of the data protection act.
JJB rings a bell, think I signed up for one to get a discount when purchasing a bike, then paid off in full and never used again, will have to do a paperwork search to find the account details. Many thanks, at least i know what i'm looking for now. The plan is to close all unused accounts in case they are affecting my credit score in any way.£2 coin savers club = £2880 -
having turned house upside down I am sure all documentation relating to this card has been binned along with the card. Does anyone have the phone number i need to cancel this card and know if its possible to cancel without an account number etc.£2 coin savers club = £2880

This thing with not telling the reason because of Data Protection is wrong. My understanding, after seeing several TV programmes on Debt issues, is you can apply in writing for the reason for a denial for credit - there is no Data Protection issue because it is your data! But it has to be in writing - phone Orange again and ask for the address to send the request to, they have to respond. This may flag up a reason like someone moved into the house you recently left and that persons credit history has effected yours.
Orange make the decision of accepting or denying credit not Experian nor Equifax, they just report your history. Also Orange may have there own records to base their decision on. The written reply has to explain there reasons.
