Council House Transfer

Hi Hoping someone can hlp with my query....My grandmother 5 years ago purchased her council house ( scotland) in her name and received her council discount. I myself paid for the purchase and the house has been willed to myself. The house is in my grandmothers name as i believe i was unable to be the sole owner until 3 years after the purchase but 5 years on my gran has not transferred the house to myself ( she has been on at me for 2 years to transfer the house and I have never got round to it - she is a bit frail and legally blind so she leaves that side of things up to me). I originally put the clause in the purchase to say that if my gran goes into a care home in the future than I will get my original investment in the house back plus interest if the government sell the house to pay for her care. A few questions I have are:
1.Can the government sell the house to pay for my grand mothers care?
2. If my grandmother transferes the house to my name and I am the sole owner can this be done just now or is there a minimal time before this can be done? I have heard reports of 5,6,7 years befor the house can be legally passed to myself or anyone for that matter?
3. If the house is transferred to someone else and my gran goes into a care home what happens then - can they chase my gran for care home fees?
The most important thing here is my gran.. If the house is sold to pay for care and I dont get a penny back then that will be fine as long as my gran is ok and comfortable. It may well be that she is able to live in the house for many many years to come but as you can imagine - there is a massive amount of diffrence in price of what my gran paid for the house and what it is currently worth.
What I would like to do would be to understand all the legal requirements before i visit a solicitor to get it arranged this week ( on behalf of my gran)
