Nationwide CC dispute form re XL

I phoned Nationwide CC last Friday morning asking for a refund for my XL flight, today I received a form with 9 different options to choose from, the only one applicable is:

I did authorise this item but I have not received the goods and /or services. I have tried to contact the merchant without success. (You must enclose a copy of your sales invoice or receipt, a copy of any documentation that you sent to the merchant and Liquidation documentation if applicable.)
Have I been sent the wrong form ?
there is no mention of Section 75 claims
I have tried to ring customer services but can't get through without a long wait.
Can anyone help ?

From what you say, that is the right form and you have found the correct part of it, you paid for services(flights) and you won't receive them! I doubt they will need any further proof apart from your invoice since XL claims must run into the hundreds for every credit card issuer0
