I want to set up a localhost page where I can type in the variables and have them emailed to some of my suppliers

Job Name:
Variable 2
Variable 3
Variable 4

All are text boxes

Then I want 4 check boxes when if clicked the recipient is bcc'd in an email.

Anyone wanna help me out please
