Advice needed - concrete slope?

Hi I am laying a foundation for a shed but need to put a slope in - can someone advise me, step by step, how I go about this - many thanks in advance :)

    dont you just put 2 pieces of wood down the sides, and use another piece to smooth it out too that level, and degree..
