Current account DD switching problem

Hi,
A friend has been attempting to switch from LloydsTSB to Nationwide (as there are no Lloyds branches in NI). The new NW current account has been setup and is working fine.
However there is major problems with transferring the direct debits. The Banking Code states it should take 3 working days, how does over 3 months sound?
First NW claimed they had requested details from Lloyds, but Lloyds say they never received them.
Then quickly NW said they made a mistake and forget to make the request (somebody went on holiday in the local NW branch or something) but have now made request, yet Lloyds still say they haven't received the request.
Who is fibbing now?
Apart from making manual DD transfers, any way of sorting this mess out?
Do the Banking Code people or the FOS need to be consulted?

The banking code only states that your current provider will send a list to your new provider within 3 days of them receiving the request. According to Lloyds they've not received this request. Even once NW receive the list they then contact the actual other companies involved to request they amend the DD to the new account..0
