Hi, Does anyone know where I can get a "Financial Loss Claims Form" from BT?

    depends on what you are claiming for and why - search BT website for their service guarantee and this should detail it for you!

    if yoou are a residential customer, call residential customer service and ask for AFL pack they will send you one which you need to fill before any financial loss can be paid to you.
    Though the vision may not be the true future of man but it can't be completely beyond his control.
