fuel charge reclaim from Virgin & BA

Had an email this morning regarding this with what was I think a claim form. It only allows claim for one and there were 4 of us which I paid for. 2 were the kids. Anyone know how to fill these in. I thought about ringing Virgins helpline as I have three years worth to claim.

As per the email, just submit the claim online, I'm assuming you are an FC member in which case that will pull through all your flights you've done.

I've just received an email too - I'm so happy I can claim just by putting in my Executive Club number rather than listing all the flights.

Does anyone know if there's anything that says how they work out how much refund you get? All I can find is that it will be £4 to £20 per flight, but not how that's worked out.

Also, I'm trying to fill out the details to have the money sent to my bank account. I have all the details except ABA routing number and Bank contact - do you think that will be ok to submit without them? I would have thought they should able to pay into my account with either the BIC/IBAN or the Account Number/Sort Code.
