Help Needed Please-medical Consent Form

My son is going to stay with his grandparents in the States this summer and my stepmum wants me to get a medical consent form for him with them listed as being able to agree to treatment, if needed. They are both US citizens and he is not, so they are worried that they will not be allowed to do so. I have trawled the net for the last hour but can't find any that are appropriate for UK to US travel. Has anyone got any good sites they could recommend?

just download any one you like the look of can find from the web- there are loads as they are required for any participant in US summer camps.
sounds like it's a good idea too in case anything happens.
this one has 2- one for medical history and one autorization form which look good.

(just make sure you are authorizing his grandparents (and not a camp))

Guess a printed form is a good idea but some years ago when our daughter aged 15 went to a family wedding in US with her grandmother we just wrote a letter giving full names addresses of both her parents and 24 hour contact details. The letter authorised her grandmother to consent to any emergency medical treatment during her stay in the US.
