Does anyone know what the rates for using a Marks & Spencer master card to get cash is the USA is?
I hear that their rates for changing money here is about the best at the moment.

Assuming you mean their Mastercard I would imagine they apply a loading for overseas transactions as per almost any other provider (except the usual suspects of Nationwide, Post Office, Saga, etc.).

Their rates for changing money, as in ordering foreign currency before you leave, are pretty good (they will put it through as a purchase rather than a cash advance transaction).

The same as any other Mastercard. Mastercard set them, not M&S. As KTF says, they will then apply a loading just like nearly every other Mastercard provider, the Post Office being the only one I can think of offhand that doesn't.
Not a good idea to get cash with a credit card though. There will be a cash advance fee plus interest as well as the loading0
