Sponsorship letter template to send to companies?

Does anyone know of any sponsorship letter templates that i can send to local businesses asking if they would sponsor my partner for a 10,00ft parachute jump he is doing for Great Ormond street hospital in August or have any ideas of what i can write in the letter?
