Id put first ever.
You are advising someone to lie on a passport application? That's not going to look to good when the name is cross-matched to the IPS database.
I would check exactly what type of passport your partner used at age 12. He may have used a group passport, or a British Visitors passport (orangy paper thing you used to be albe to buy at th ePost Office without proof of nationality, phased out over 10 years ago due to high amount of abuse) or a full child passport. Once you have found out which type of passport, then phone the IPS helpline for advice since you are not in possession of the long out of date previous passport. Don't just answer no to the question until you are advised by IPS.0
