Good & *flexible* travel insurance

We're heading off on a 2 week trip to Spain shortly and I wanted to buy an annual policy since we are certain to travel a few times over the course of the next year.
I don't want to over-cover us, but if we were to go further afield than Europe I would like to be able to extend the cover we already have rather than simply buy a new policy.
Is anyone aware of any decent travel insurance companies that would for instance, allow me to take out a Europe-only policy now, then perhaps extend the cover if/when we need it?
