Any succesful appeals after being declined for a CC?

So after being declined by Capital One for a card as their instant computer decision maker turned me down, I've decided to appeal, more so after finding a fault in my Equifax credit report.
Thing I wanna ask is, what should I put in a letter. I plan on being honest with my debts and outgoings and include 3 months of bank statements.
I'm just not sure what kind of approach I should take, as I think I need to show I'm willing to spend but also not be totally bonkers with spending.
Any help would be most appreciated.

I would advise that you contact Equifax and Experian (they are the main gossip mongerers, I mean credit reference agencies) and get any incorect information put right. It is unusual for Capital One to refuse outright, more often than not they will ask for a security deposit, which I beleive is refunded after 6 months satisfactory conduct.Don't lie, thieve, cheat or steal. The Government do not like the competition.

I'm sorry, I don't apologise. That's just the way I am. Homer (Simpson)0 -
I've done an online dispute form with Equifax, so *hopefully* that will clear things up on that front.0
