Surely if the OP had given the wrong address intentionally, he wouldn't have 1. chosen his own street and 2. immediately corrected the house no.?

Thanks for sharing that, why post such utter drivel?

OP, as already mentioned above, go to Rail UK Forums and post the exact same thing there. They will give you outstanding advice without judging you, (unlike 90% of the pillocks on here).

No 15 instead of No 11.

I was flustered and embarrassed with others looking on and the inspector asking me questions, I just cant account for the error. I did immediately correct him, but that was not until he had checked and questioned me over the address.

I maintained my explanation for the wrong address in the statement the inspector recorded, it is the truth that I wasnt seeking to provide false information. In addition I gave my correct full name which id guess is unique.

Really appreciate the help people have given, currently awaiting the letter from First Capital Connect and hoping they'll realise the impact this could have.

So you thought that the 17th of June was the 15th, and that some how confused you so much your house number became 15 in your mind as well?

I get my house number mixed up all the time. I used to live at a 24d and now I live at a 23a and my brain still tries to say 24d. Then a while ago I ordered a parcel off the internet and somehow typed in 22. I have no idea how or why, I never even noticed until my boyfriend pointed it out when I showed him my order. I had to post a note through number 22's door explaining how stupid I am, it was embarrassing.

I also forget my own phone number, because I never phone it.

I also get the month and day of my birthday the wrong way round when I'm writing it in number format, because the numbers are next to each other (eg 11/12/yy when it should be 12/11/yy).

So I for one believe the OP!

I have. Especially when I moved I ended up getting all my mail and delivieries sent next door. Still happens every so often now too when I enter their number in the box.
