address to send black horse ppi claim?

Hello, i have just filled out a ppi claim for a loan i had through black horse, does anybody know the address i should send it too?

Photocopy and scan the form plus your evidence and then send it to the bank by post first (always better in writing I reckon) - you can get a free proof of posting though you may want to do it recorded just in case.
