PPI and credit card charges…HELP!

I’m re-claiming ppi on a credit card (on behalf of girlfriend) and have just had word back from the company involved (Lloyds tsb). They have declined the claim as there was a box to tick on the form to ask if she wanted ppi. This box was ticked (by the salesperson). The only problem is that she never actually filled in the form (the salesman did) and she just signed the form without checking (silly girl!). What comeback does she have in this case? It is not her handwriting on the form but how can she prove it? She’s probably due close to £1000 so want to keep going. Do I just go to the FO?

(Background: I had said in initial letter that the salesman was not qualified, she wasn’t aware that it was optional at sign up, and had just realised (I realised!))

trying to get credit card late payment charges back as well (with lloyds). Sent letter asking for £225 in charges and £50 interest. This was made up of 8 or 9 £20 charges and 4 £12 charges (post june 06). They use the old argument that the £12 is ok (but will pay the difference) and have sent a cheque for £51 which includes court interest on top (but don’t actually breakdown what charges relate to the £51). The letter with the cheque said it was a final offer and they have closed the case. They included ombudsman leaflet. What do I do? Can I cash the cheque but say this is part payment and take the matter to FO? Or do I just go straight to FO? Is it worth pursuing the case, what are my chances of getting the full £275?

any help greatly appreciated ( I can provide more details if necessary)
