Normally that keeps happening if either the card (likely) or the reader (less likely) are faulty.

Sounds like the card is faulty.

File format FAT32, NTFS makes no difference.

Windows not being able to format usually means the cards kaput!

Is it an SDHC or SDXC card?

My laptop (a two year old HP Envy i7) will throw that error if i try to insert an SDXC card, as it only can read SDHC cards.

Does the card otherwise work on / in other devices?

I've had problems formatting flash USB devices in the past. This HP tool has fixed the problem for me:

Will have a look thanks. My laptop has no issues at all with USB sticks. Its SDXC that it objects to. Will see what this tool makes of it.

. The virus will keep writing dangerous files to it, leading to the SD card inaccessible.

    That sentence is nonsense

Edit : after noticing the last sentence - whole post marked as spam

The SD Association have a format utility in the Download section:
    https://www.sdcard.org

I use their Windows utility program occasionally to clear partitions from SD cards when working with a Raspberry Pi. Removing a disk partition from an SD card can be tricky under Windows, this utility does it while formatting.

Also, note there are a lot of fake cards around...
