I got this as well, don't remember filling it in either! - must be old age ;D
Also got a £4 luncheon voucher from pinecone research, and then they sent me another one the next day - whoosh!! - also got a card thru' me letterbox, the post office couldn't deliver a parcel as it was too big - really not got a clue what that is, gonna have to wait til Friday before I can pick it up
