From 1 January 2006 the E111 form has been replaced by the European Health Insurance Card (EHIC) so if you are a UK resident, you can apply for an EHIC by any of the means shown below.
The EHIC entitles the holder to free or reduced cost, state-provided medical treatment if you fall ill or have an accident while visiting any country in the European Economic Area (EEA). Please note that an EHIC is not a substitute for comprehensive travel insurance. If you claim medical expenses under your Post Office travel insurance policy and you use your EHIC entitlement, your policy excess will be waived. Check out our great value Post Office travel insurance.
All you need to do is fill in the form and you will receive the your EHIC,valid 3-5 years. You will find all the useful informations on the site
I hope this is useful for anyone who is travelling to Europe

This is vital for traveling to Europe, please don't leave the country without it in your wallet! My girlfriend went on holiday to Prague without her EHIC or travel insurance (doh!) and got appendecitis - turned into a lot of trouble and expense.
In the event that you forget/lose your EHIC and find yourself in need of medical care abroad, you'll need to get someone to contact the DWP with your details (including national insurance number) and the details of the hospital where you're receiving care. They can then fax a Provisional Replacement Certificate to the hospital, which they can treat the same as your EHIC. Found all this out the hard way :eek:0 -
I always keep mine with all the appropriate documents so I don't forget to bring it with me.0
