advice relating to CCA

I have decided to write to the DCA's with regards to the CCA of the debt they refer to. i took all my debt out in 2002 and havnt paid much of my debt for the last 3 years. (7500 personal loan halifax, 4000 personal loan natwest, 2x 3000 credit cards halifax)
for the last 3 years i have recived many letters from different DCA's wanting thier money. tho never knocked on my door or arranged to go to court!
All of the credit was took out in local branch in 2002.
i have downloaded a basic template of the 1st letter to send them requesting a CCA.
my query is
a- ) in the letter do i refer to the debt as mine? do i acknowledge it?
b- ) im printing it off the comp ..is it right that i dont sign my name on it?
c- ) what does the 12 +2+30 mean?
i get the gist of the have 12 days to reply what after then?
is there a follow up letter i then send?
also what if no-one has a CCA what happens with the credit... does it stay on my file forever?
all your advice would be appreciated i plan to send the letter of later today.

Let me start by saying that if you took the loans out at a branch , I would expect all the paperwork ie the CCA to be correct . Having said that it could be it has been lost in the filling system.Or has not been forwarded to the DCA. POINT a You do not have to acknowledge it .
b do not sign it .
c they should send a copy within this time limit , if not they have committed an offence .
follow up letter around somewhere !0 -
so basically they most probably got a cca at halifax for me?

No, it does not help really . Just wait and see if a CCA is produced . If you can not afford to repay the amount they want and it goes to court . Do a SAS COST £10 , but they have to provide the last six years of statements . Add up all the charges and put in a counter claim to reclaim charges .0
