They've already given you the job so I think they would be on -very- sticky grounds if they now dismissed you on the basis of something you put on your form at this stage. It's probably just for their records. I'd fill it in honestly.0

They wouldn't be on sticky grounds to dismiss at all. Honesty is a basic requirement. OP you really need to disclose everything as you have been asked, and you've already come to that correct conclusion.
Also please bear in mind that it can work in an employee's favour on occasions; for example, if you have suffered from a condition which falls under the DDA, or may do so in the future, if you have disclosed this to your employer then regardless of anything else they are deemed to have accepted the implications of the condition, regardless of the amount of time you take off. If you lie you do not have this protection.
For example, an employer can still dismiss someone who falls under DDA due to the amount of time taken off, but it might take around a year with an employee with some length of service eg a year or so; if that person has been taken on with the knowledge that they suffer from a problem which may result in time off the employer will find it very difficult to get rid of them as the assumption is that they knew exactly the problem when they took them on, so what has changed?0 -

Congrats on the new job. You know, I wouldn't worry too much about this.
I've recently got myself a new job and had to fill out an occupational health questionnaire. I suffered from post natal depression for about a year and said so on the form. I got through no problem, no further questions asked.
I think you've no other option but to be honest - but don't give any more information away than the form asks for. God forbid if you took ill in the future, and they found out you had not disclosed an illness then that wouldn't be good!
Remember you've already got the job - it costs an employer a fair bit of money to recruit people, as well as the time delay for getting someone to start so its really not in their interests to dismiss you unless on strong grounds.
Just my opinion of course, and I'll keep my fingers crossed, but I reckon you'll be ok.0
