Debt Management Plans advice needed urgently

Hello Board,
I am a newbie but I am in a lot of debt.
I'm 24 and currently owe roughly £28k. It has got to the point that I am paying out more than I am earning so I need to do something about it and need to do it quickly!!
I have been looking at one company and they are recommeding that I go on a Debt Management Plan, I am not 100% sure what this means so some advise on that would be great.
Also they are charging 15% which has been included in the monthly payments but this seems very high is it?
I would greatly appreciate any information/advise that people are willing to offer to help me get out of what seems like a no win situation with my finances.
