What tax are you thinking you can claim back? Are you contributing to a pension, or do you beleive that too much has been deducted as PAYE?

If you are talking about the year 07-08, no form necesssary, just send a letter to your tax office, with your P60, saying you believe you are due for a refund of tax and the reason why.

Better make sure before you do anything - enter your figures in http://www.listentotaxman.com/

Before you send in your P60, it's worth phoning to see exactly what details are already held. if HMRC have all the details needed to assess the year for a repayment they will do this from the phone call, if you have to send in letters it will take longer because most offices have post backlogs, not to say that an assessment done over the phone WILL be quicker, just you wont have to wait through the post backlog if they have all the information needed.
Nothing's free.....but we'll see what we can do!
