CCA Request,step by step

Hi,I've applied for CCA's with 5 different companies,and so far have received not one CCA,had letters saying they are looking into it,but nothing more,what I need to know is what the exact steps are now?
What do you do after the 12+2 days have passed,do you send them a letter,or do you bide your time?If so what happens then,what letters do you send after the 30 days are up,also is it 12+2+30 or 12+2 then a total of 30?
I have looked through the forum but can't find a definitive answer,so any help would be welcome.
