Just check the old card number with the new one and the 2 should match, they've updated the look of the cards,

But stick the old card in the box in store and they give you a bunch of points. So they said.

It is just a replacement for your old Clubcard. Once you have activated your new Clubcard by using it in the store the old one will not work and there is a recycling box in store in which to put your old card (which then gets entered into a draw to win 1000 Clubcard points). The latest Clubcard vouchers sent this month are advertised to only work with the new card number.

You do not need to do anything about your credit card, which will still be linked your Clubcard account.

I thought I linked my clubcard to my credit card, but I still ended up with two clubcard accounts, sounds like they've done the same with yours.

Not complaining though, they keep sending me vouchers on my ordinary clubcard as well as my credi card and every now and then I get X off X spend, so I would say keep things as they are, you get more vouchers. I use my ordinary one for small cash spend and my credit card as my normal credit card (weekly shop & everything else) !!!!!!!!!!!

AFAIK they never used to expire, but now the quarterly vouchers issued for points expire 2 years from issue - the expiry dates are printed on them, though perhaps actually you are asking about the actual points rather than the vouchers? Sorry, can't help you there.
