changing PPI?

My brother and his wife have a loan with Natwest which includes PPI. They are happy to have PPI on their loan but the amount is putting a hefty figure on top of the original amount, and has hiked the monthly payments up . He would like to know if they would be able to cancel the PPI with Natwest and take out PPI with another bank/financial institution. If they can, can someone please point me in the right direction of who would cover the loan. The PPI they have at the moment only covers sick benefit for his wife as it is a joint loan but also carries 5k death benefit. The loan has 3 years left to run.

If it is a personal loan then normally the first named is covered only. Who is the main earner here?
I don't think another bank will take on the PPI but they can get policies in their own names on the open insurance market.
