I tried self employment from 1st Feb to 5th April before and after that on JSA.

Soon after starting I told IR that I have started a SE business and they sent me the form to declare you have started. By the time I got round to completing it I could see the business wasn't working so ceased trading just before I sent it back, explaining (in the box for any other info) I started on 1st Feb and finished on 5th April and didn't make any profit.

I haven't herd back from them at all, I hope they just opened and then closed it.

Do I have to fill in a self assessment or anything else as well?

yes, you will have to self assess, showing your nil profit (or loss). The form should be with you soon.
